千 is a thousand: a tally stroke (一) drawn across a figure (人), which lent only its old sound. 萬 is ten thousand: a borrowed scorpion graph, kept for the number alone. The two biggest counting words stacked carry no count here — they pour their whole magnitude onto the instruction behind them.

千萬 weights an instruction to the listener — be sure you do this (千萬要小心). 一定 marks certainty about a fact — this surely is / will be so (一定會下雨). One leans on a command, the other predicts an outcome.
別 plainly cuts off an action (別忘了 — don't forget). 千萬別 keeps the cut and stacks the speaker's full weight on top (千萬別忘了 — whatever you do, don't forget). Same prohibition, urgency added.
千萬 before a bare verb: 千萬記得帶 → 千萬要記得帶（千萬 leans on a modal 要/別/不要, not straight onto the verb）
千萬 for a sure outcome: 明天千萬會下雨 → 明天一定會下雨（千萬 weights a command, not a prediction）
reading 千萬 as the number here: 千萬要小心 is 'be sure to be careful', not 'ten-million-fold careful'（the adverb drops the count）
English folds this into 'be sure to' / 'whatever you do' / 'make sure you don't', which sit anywhere in the clause; learners drop 千萬 in late or attach it to a noun, or confuse it with 一定 since both translate loosely as 'definitely'.
